Dr. Dunn is also a Principle Investigator with Delta Research Partners of West Monroe providing patients with access to developing treatments.

Please complete all paperwork if you are a new patient or if you have not seen Dr. Dunn within the last year. All paperwork must be completed prior to your procedure or appointment. If you are scheduled for an appointment or procedure and would like us to text you the paperwork, please call our office. We utilize Klara which is a HIPAA secure text messaging platform. This text messaging platform gives the patient the ability to fill out and submit all paperwork on their phone/computer without having to print.
